Kolbi Brost, DC. DACNB. (Doctor of Chiropractic & Diplomate of The American Chiropractic Neurology Board), has been in practice since August 1998. She has been the heart of Brost Chiropractic & Wellness Centre since September 2003. First Visit - Initial Paper Work
Upon entering the office, we have some paperwork for you to fill out. The paperwork provides us with thorough information about yourself, your condition and history. Please call 403-520-5265 to find a suitable time for your New Patient Exam.
Please choose an intake form based on the patients' age:
Child 6 - 15 yrs Initial Questionnaire
Child 0 - 5 yrs Initial Questionnaire
For those with Extended Benefit Coverage, please download, print off and fill in the form below. This will help us determine if we can direct bill with your insurance company.
IMPORTANT: If you have been involved in a Motor Vehicle Accident please email us for all the forms that you will need. This will save time for you in the office. We are also a WCB approved centre. Please let us know before your appointment if you are claiming WCB.
GUIDE TO CARE AFTER A CAR ACCIDENT.
Please bring along your ALBERTA HEALTH CARD and if relevant, any Extended Insurance information you may have. We offer direct billing as a courtesy to our patients. If your extended benefits does not pay the full amount, or we are unable to direct bill, you will be responsible for payment. We take Cash, Debit, Visa & MasterCard.
